Verification of Training Requests

Request(s) for Verification of Training Information

The following process will assist in expediting verification of training requests.

All verification of training requests must be submitted in writing and must include an Authorization for Release of Information and a Verification of Training Form.

Please note: ‘Loss History’ requests are considered ‘Professional Liability Insurance coverage’ requests. The requestor is responsible for emailing the credentialing office to obtain this information at: malpracticeverification@ttuhsc.edu
