Employee Medical Records

The employee’s occupational health medical record is maintained in a separate and distinct location from the other clinic medical records. The occupational health medical record is treated with the highest level of confidentiality and respect for employee privacy. 

Employees can request a copy of medical records in person at 4801 Alberta Avenue, Occupational Health Department, located in the basement of the Clinical Science Building (CSB), Room B04. Bring your employee ID.
