Health Care Provider Clause

When contracting for a physician, nurse, or other health care provider services with an individual or entity who will provide health care services to TTUHSC, add the following:

[Agency] will provide proof of current licensure and certification. [Agency] will notify TTUHSC immediately of any change in the status of the licensure.
