Referral Management Process & Ordering Referrals:
In an effort to streamline the referral management process, the previous method of specialists ordering referrals
as an “Order” is being phased out. You will now see the form below in your office visit to create a printout with
information and instructions for your patient regarding any referrals you may suggest the patient obtain from their
primary care provider or directly through the specialist if the patient does not have insurance requiring a referral
from their PCP to see a specialist.
If a provider has a patient that requires an evaluation or further assessment, please use the form below. Once
this is done, please click the orange button to print a handout for the patient, as well as copy the information
into your patient instructions.
A member of the CIS team will be visiting clinics to give more information and answer questions on this change.
