Before filling out this form, please browse through 3-D print designs page. Once you find a design you like, please copy and paste the link into the “Description” portion of the form. Please only submit one request per form.

Copy and paste the link for your print here. If you have your own design, write “own design” on the form and email the file to elp3dprint@ttuhsc.edu. If the file is too large, stop by either library with the file on a USB.
