COVID-19 Survey
An Internet Survey on COVID-19 Presentations and Outcome
The COVID-19 pandemic is the defining global health crisis of our lifetime. We are collecting information from patients with COVID-19 infection to assist with our strategies to avoid or mitigate subsequent waves of virus transmission. This survey will include adults ages 18 and older.
The survey should not take more than 10 minutes to complete and your responses are completely anonymous.You may skip any question you are not comfortable answering. Your participation in this study will have no effect on your medical care. By responding to the survey questions, you are consenting to participate in this research study.
